Detoxification Services and Medical Residential

Adolescent detoxification treatment services at Kairos Healthcare are available to adolescents ages twelve (12) thru seventeen (17). Kairos ascribes to the principles of detoxification set forth in a variety of research sources which puts forth the following principles of detoxification:

bullet Detoxification alone is rarely adequate treatment for AOD dependencies.
bullet When using medication regimens or other detoxification procedures, medical staff should use only protocols of established safety and efficacy.
bullet Providers must advise patients when procedures are used that have not been established as safe and effective.
bullet During detoxification, providers should control the patient’s access to medication to the greatest extent possible.
bullet Initiation and management of withdrawal should be individualized.
bullet Whenever possible, medical staff should substitute a long-acting medication for short-acting drugs of addiction.
bullet The intensity of withdrawal cannot always be predicted accurately.
bullet Every means possible should be used to ameliorate the patient's signs and symptoms of alcohol and other drug withdrawal.
bullet Patients should begin participating as soon as possible in follow-up support therapy such as peer group therapy, family therapy, individual counseling or therapy, 12-step recovery meetings and Alcohol and Other Drug recovery educational programs.

The Kairos Adolescent Detoxification Unit meets the standards set forth by Blue Cross Blue Shield of Michigan related to medical detoxification in a free standing facility which include:

bullet Physical exam within 24 hrs. of admission to include but not limited to DSM-IV R diagnosis
bullet Medical follow up weekly by Physician or Physician Assistant
bullet Nursing care – 24 hours per day, 7 days a week
bullet Supportive therapy and counseling
bullet Individual, group and family counseling
bullet Transition to continued care according to the needs of the consumer
The Kairos triage process developed in conjunction with the Kairos Medical Director includes all of the following:

· The method used in determining the level of urgency of need of each individual client.
· Identification of the services to be performed, including transportation if necessary.
· The method of assigning the priority of required services

Residential Substance Abuse Treatment - Detoxification is just the first step in the recovery process. Many adolescents will require a residential course of care if they are to remain alcohol and/or drug free.

Kairos Healthcare provides two residential care models following detoxification. A medical model is

available for adolescents whose health care insurance mandates this model. Our therapeutic model has been in place since 2000 and is currently available to adolescents and their families.

Adolescents residing in the Saginaw area continue their outpatient treatment at Kairos following residential care. Kairos has developed excellent relationships with outpatient providers throughout the state when the youth does not reside in the Saginaw area. Outpatient appointments are made while the youth is in residence.

Motivational Interviewing - It is not uncommon that adolescents are un-motivated for treatment when they present for detoxification and/or residential treatment. Kairos therapists, counselors and direct care staff understand this fact and use principles of motivational interviewing in relating to adolescents at all levels of care at Kairos.

The application of motivational interviewing is a primary intervention that helps the clinical team in avoiding confrontational styles of treatment. It forms the basis of a treatment approach guided by five principles: (1) express empathy, (2) develop discrepancy, (3) avoiding argumentation, (4) roll with resistance and (5) support self efficacy.

Co-occurring Substance Abuse and Mental Health Issues - The majority of the adolescent patients presenting for treatment at Kairos Healthcare suffer from co-morbid substance abuse, mental health and behavioral issues. Based on this fact, Kairos has staff, programming and services in place to meet the needs of the dually diagnosed patients referred to our facilities. This is evidenced by our track record and the granting of licensure as an Integrated Treatment Program issued by the Michigan Department of Health, Bureau of Health Systems.

All assessments are performed by clinicians who are Master’s prepared or higher. Therapists assessing dually diagnosed adolescents must have at least five years experience in mental health and substance abuse treatment. Physicians and nurses are professionally prepared and experienced in working with youth who suffer from both substance abuse and mental health issues.

Family Involvement - Kairos mandates family involvement. This includes family therapy sessions as appropriate during the adolescent’s detoxification/residential/outpatient course of care. In these family sessions, each clinician is entrusted with the task of helping the family to understand the effect of the family on the adolescent and the effect of the adolescent on the family. When this is accomplished, understanding and family goals can be established and processed over time.

Kairos Experience - Kairos opened outpatient clinics in Saginaw and Flint in 1997.  Our adult residential facility in Saginaw was opened in 1999. Adolescent residential treatment services were added to the campus in June of 2000. The adolescent residential program grew and our present Family Care site was opened in 2003.

Kairos is experienced with the un-insured, under-insured and Medicaid populations managed by Pre-Paid Health Insurance Plans and Coordinating Agencies throughout the state. As of this writing, Kairos is contracted with twelve (12) PIHP’s and CA’s to deliver adolescent residential substance abuse treatment. Kairos also accepts most commercial insurances.

It may occur that patients referred to Kairos Healthcare for detoxification may require medical or psychiatric hospitalization. Kairos has an excellent track record working with Covenant Healthcare when medical hospitalization is required. We also work closely with Healthsource of Saginaw when a youth requires psychiatric hospitalization.
